Monument Dermatology, PC ("Monument," "we," "us," "our") is committed to protecting the privacy of patients and visitors. This Privacy Policy explains how information is collected, used, and shared when you visit monumentdermatology.com, contact our office, or receive care from us.

1. Health information — HIPAA

As a covered entity under the Health Insurance Portability and Accountability Act (HIPAA), Monument maintains a separate Notice of Privacy Practices describing how protected health information (PHI) is collected, used, and disclosed in the course of providing care. A copy of the Notice is provided at your first visit and is available on request.

This Privacy Policy describes information collected through our website and general office contact — not PHI created in the course of treatment.

2. Information we collect online

When you visit monumentdermatology.com, we may collect:

  • Information you provide voluntarily — name, email address, phone number, and message content — when you fill out the contact form or schedule a visit
  • Standard server-log information — IP address, browser type, pages visited, and timestamps — collected by the hosting provider for security and analytics
  • Cookies used by third-party services we integrate, such as the EZScheduler patient-booking widget and embedded maps

Information collected through the website is used to:

This site integrates third-party tools that may collect information independently:

  • EZScheduler / EZDerm — patient-facing booking widget. EZScheduler's privacy policy governs information you enter into the booking flow.
  • Google Maps — embedded map. Use of the map is subject to Google's privacy policy.
  • Google Fonts — typography. Google's privacy policy applies.

6. Cookies

Monument's website uses minimal cookies — primarily for the embedded booking widget and map. You can disable cookies in your browser settings; some site features may not work properly if you do.

7. Security

We take commercially reasonable measures to protect information transmitted through the website. The site uses HTTPS encryption. PHI created in the course of care is stored in our electronic health record (EHR) with the security controls required by HIPAA.

8. Your rights

9. Children

Monument provides care to patients of all ages. Children under 13 do not interact with the website directly; appointment scheduling for minors is handled by a parent or legal guardian.

10. Changes to this policy

We may update this Privacy Policy. The "Last updated" date at the top of the page reflects the most recent revision. Material changes will be noted on the home page.
